Hospital Admission Risk Program
The Hospital Admission Risk Program (HARP) provides short-term care coordination for people with complex medical and/or psychosocial issues.
Who we care for
We support people who use hospitals often or are at risk of being hospitalised and may benefit from support to self-manage their health and remain independent.
HARP specialises in assisting people with:
- Unstable diabetes
- Chronic respiratory disease
- Chronic heart disease
- Complex psychosocial concerns
- Complex aged needs (including falls)
To be eligible you must be 18 years or older.

- Horsham City Rural Shire
- Northern Grampians Shire
- Pyrenees Shire
